The most exciting part of this whole thing was putting my hair up and going to the gym and not being self conscious. Before, I felt like my nose really stuck out, especially when I put my hair up. In closing, when I look in the mirror, I look at photos. It’s funny because people always have this notion in their head that cosmetic surgery is going to change their face, change them completely.

First of all, it’s why you have to find the right doctor, and I knew Doctor [Sinha 00:00:21], he told me that he would give me a nose that looked like me. I said, “I don’t want you to go to natural because I want it to look better than natural.” He goes, “No. I’m going to give you a nose that is perfect for you and people will just think you were born with the perfect nose.” I said, “Oh, I like that.”

So I actually had a patient at work one day. She said to me, “Can I ask you something? Have you ever had a nose job because you have the perfect nose?” I said, “Oh, that’s wonderful.” In my head, I was just so excited. I was like, you just made my day and I’ve heard other comments like that to you but honestly, it just feels like a part of you because I think when you have something you’re self conscious about and then you correct it, you feel like you look like the you, you’re supposed to look like. The you, you envisioned; a better version of you.

So it hasn’t been hard to get used to or anything. It’s actually like, “Oh, this is how I was always supposed to look in pictures.” So just a better version of yourself.

A lot of people come and say, “Hey, what are the types of rhinoplasties you do?” I think that’s a little bit of a flawed question because as I mentioned earlier, there’s a lot of different structures, nine plus the skin, which create the one structure that people see, and that’s the nose. If you think about all the different combinations and variables they can be, there’s not just three nose types, the nose with the hump, the droopy nose, the wide nose is sometimes a lot of places just try to put things in buckets.

I think every nose is individual. No two noses look alike, because unless I get a pair of twins, no two people are alike. Each nose is customized for that individual. Again, it goes back to what are the wants and desires of the patient? What is the history of the nose? Was it operated on before? Was it hit with a softball during high school or an elbow during a basketball game? Maybe this is a nose that is a family nose, and there’s lots of great things that run in your family, and there’s some things you just want to change. Maybe it’s the nose.

We take all the circumstances and individualize and customize your approach to each person based on all of these things. Every nose is the nose for that particular patient.

So the ideal candidate for a rhinoplasty is somebody who has a great motivation for doing the nose. And what is that great motivation? They want to be the best version of themself when it comes to the nose. You know, they don’t want to be somebody else, they’re not doing this because it’s kind of self-inflicted negativity, et cetera, they’re like, “Wow, I’m good, and now I want to be great. And one of the ways I’m going to get there is if I can take this hump or this crook or this twist out of my nose.”

Often, it’s occurred from, say, perhaps sports in high school, or something that, when they look at it, it kind of makes them look at themselves in the mirror and say, “The person that I feel on the inside is not the one that I see in the mirror, and this nose is what’s detracting from it.” So again, we want people that do it out of a self-confident state and who want to be more self-confident.

Hi, my name is Lindsey and I received a septorhinoplasty from Dr. Sinha in 2017, the very end, the year, and I came upon Dr. Sinha by just doing research. I had had nasal surgery in the past when I was a young teenager, and I was not old enough to receive a septum straightening just yet, just because I wasn’t yet an adult. I knew that down the line, someday I would want to do that. I came upon Dr. Sinha by researching ENTs that also specialize in cosmetic surgery, and they were able to get me in right away. It was very easy process. Upon the consultation, he was so friendly and very relaxing. He just has a way of putting you at ease, and I am one that I need that because I overthink everything.

So the initial consultation was wonderful. He has an excellent, excellent bedside manner. He makes you feel really comfortable. He’s very detailed oriented, goes over every single part of the procedure, but also, he’ll take pictures and he’ll show you what your nose actually looks like, which is great to be able to see your before and after, before you’ve even had surgery.

It had a small hump in it that I didn’t like. I don’t know how much other people noticed it. I didn’t like it, never liked it. Hated my profile more than anything. From the front, wasn’t that bad, but I hated for anybody to take pictures or anything from my profile. I couldn’t stay in my profile.

For me, also, he recognized that I had a deviated septum. At the consultation. Doctor [Sinha 00:00:03] was able to take photos of me using the Vector photography system, which is an amazing tool that they have at their disposal. He takes photos of your face and they put it on their flat screen, HD TV, which is awesome and they can show you, he shows you with his little tool, what exactly he can do for you and then they actually send you home with your own copies so you can put it in your folder and look at it of how exciting it is of what you can look like.
They do three different angles as well. So it’ll show you a split screen of the before and after. So it just gives you a general idea of what they can do. It’s not going to be exact, but it’s a strong idea of what it’s going to look like and it’s actually comes out even better than what they show you.
So it was a really cool extra tool that they had and I think every office needs to have that. We had really nice consultation. He took the time with me, answered many of my, all of my questions cause I have many of them and yeah.

Rhinoplasty surgery is performed utilizing concealed incisions inside the nose. In some cases, a small “w-shaped” incision is made in the skin of the columella (the structure between your nostrils). Lots of patients wonder about the experience on the day of surgery.

In our surgery center, one of the things that we are very lucky to have is our own private ambulatory surgery center. Fully licensed, accredited, so all the things that you would find in a hospital but in a very private environment, almost like a boutique hotel. A lot of the patients are surprised when they walk in that they’re the only patient there. That means the entire staff has one mission and that’s them. They’re not part of a herd of patients coming in for surgery like you would sometimes see in a hospital or busy outpatient surgery centers. That’s, I think, one of the things that really gives the patient a lot of comfort.

We have some time before the surgery, during the surgery, and after the surgery. So patients generally arrive about an hour to an hour and a half before the surgical time so they can come in, they can see the facility, they can change, and they can kind of get comfortable in their room. We have licensed RNs with lot of surgery center experience that are usually there. They make the experience very nice, make the patient feel comfortable. Even the one thing that people dread that’s the IV start is done in a very safe and comfortable manner.

You’re going to be hooked up to monitors and get some .. the nurses ask a lot of questions to get the patients ready for surgery. It’s also a time to meet the anesthesiologist. Most of the patients have met me, they get to meet the other MD in the room. We only use board-certified MD anesthesiologists just to maintain a very high level of care. When patients come in, they have two MDs, two Rns, and two scrub techs focused only on them. Again, it’s one of the things that we’re proud of in our ambulatory surgery center.

People get medicine to relax them and they’re generally very calm and relaxed by the time they go into the operating room. Once they enter the operating room, it’s usually an interesting place. There’s lots of monitors, there’s machines. There’s certainly a lot of people that are there to take care of the patient and so we are very focused on making sure that transition is comfortable. Patients are given a nice warm blanket, they’re laid on a nice, comfortable bed, and the anesthesiologist will generally do an excellent job at getting the patient prepared for anesthetic.

Usually we have fun, we have a countdown and people start, “Ten, nine, eight, seven,” and generally they say “six” when they’re in the recovery room. It’s a very quick sort of thing and they don’t realize that between seven and six, we got a whole procedure done for them. When they wake up out of anesthetic, you know, a little groggy but the anesthetic we use allows people to emerge very quickly and very easily. Nice soft landing, you know, after a nice plane flight.

The anesthesiologist is there. We have RNs again and of course, I come and check on the patient to make sure they’re doing well. Obviously, their surgery, there may be some pain, but we have excellent pain relief both through the IV line and also oral medication as well. There’s actually not a lot of pain because of surgical technique. When I operate on a nose, you know, we respect, without getting too technical, tissue planes. We’re not rough housing in there. We’re very smooth and very technical on how we do things and that results in very minimal tissue damage and therefore less swelling and less bruising and less pain.

The day of my surgery was so exciting. I won’t ever forget it. My mom actually came in town to take care of me because I knew that I would need some extra help the first couple of days, so she put me at ease. Definitely I recommend to have somebody you trust with you.

I could not get in that office fast enough. I could hardly sleep the night before because I was so excited. I walked in and she’s like, “Are you nervous?” And I said, “No, I’m excited. I’ve been waiting for this since I was 14 years old.” So, I think if you really let that excitement and the visualization of what you can have, that just trumps all of your anxiety. So, that really helped me a lot.

My mom also got a very funny video of me before I went in because they give you some medication to help you relax, and it was just a really easy process. I mean, even just putting my gown on, and putting the little blue socks on was exciting for me. So, I just kind of soaked up every moment and enjoyed it, and plus, I like to be pampered and I like attention, so that helped too.

So, day of surgery, my husband brought me in. To be honest I was at that point not nervous about the procedure at all. Dr [Sinha 00:00:17] had explained it all in detail, all the potential side effects, everything else that could potentially happen, and I was just excited to get it done. I don’t remember any pain or anything else during it. It was awesome. Even when I went home that night, no pain, no nausea. I slept a lot. That’s basically it.

After rhinoplasty (nose surgery), the nose is taped with a nasal splint to hold the support structures in place. Absorbable sutures are used inside the nose. If external incisions are made, there may be sutures that need to be removed on the skin.

There are no visible scars after you’ve recovered from a nose reshaping surgery. All sculpting and reshaping of bone and cartilage take place inside the nose. A small sponge is placed in the nose to prevent bleeding and can be removed 1-2 days after the surgery. The tape and nasal splint are removed on day seven. Mild to moderate bruising underneath the eye for 1-2 weeks may occur. Pain is well-controlled with pain medicine, which our doctors will prescribe for you.

So the day of surgery, I was excited, but also really scared. It was really early in the morning. So when I got to the hospital, everyone was super friendly. I got the anti-anxiety medication. RHINOPLASTY – Preparing for your rhinoplasty surgery

So a couple of things patients can do to be prepared for the day of surgery is of course we read the instructions that we give. We’ve done this a lot. We’ve done thousands of surgeries over the almost 30 years, so the questions that come up and the things to expect are pretty much the same. So we’ve kind of put it all together on some documents and if the patients kind of look at that, they’ll find the experience of thousands of patients before them and it helps them understand what to expect and makes for a much more easier day of surgery.

Certainly, there’s some things we want people to avoid. Don’t eat eight hours prior to your anesthetic time. You can have some fluids, but fluids means clear water, not tea, not orange juice, not coffee, not beer, not a shot of vodka. Just clear water up to two hours prior to surgery and then you don’t get dehydrated. We want the experience to be great, but more importantly we want the experience to be safe. We always give prescriptions before the day of surgery. It’s just one less thing you have to deal with and we try to get all the paperwork and all the other incidentals done prior to surgery.

So when it’s the day of surgery, we’re focused on one thing and one thing only and that’s so the patient can have an amazing experience. Patients often bring family members. We have a nice waiting room for them. We also want people to be aware of what to expect after any anesthesia. Most people have a sore throat. Most people get upset stomach or feel a little queasy. That’s from the anesthesia, very similar to how one may get a hangover after drinking alcohol. When your body breaks down alcohol, it’s those breakdown products that make you feel sick.

Likewise when your body’s getting rid of anesthesia, those breakdown products can cause some negative kind of feelings during the recovery period. We give medication, anti-nausea medicine, pain medicine of course for the pain. But I think an expectation of what to expect. The fact that it only is for maybe a couple of days before it starts getting better, I think, really helps patients feel good about the post-surgery experience.

The first day after surgery was pretty tough. I laid in bed for the next, I think, two days. The night was the most complicated really. But once I got through those first two nights, it was just during the day where it would hurt. I think the most uncomfortable part was just everything draining out of my nose. Once that was done, things got way better. I actually went to work after the fourth day. I was still bruised, but I worked at a doctor’s office. So people knew that you’re recovering and stuff. Yeah. I think I started getting really back into things after two weeks. The bruising kind of went down. My nose was still really swollen probably for two months. Then it went down significantly after six months. Then it just kept getting better and better. After a year, it was just like you couldn’t even tell. The most exciting part was putting on my makeup and not having that issue with my face anymore. Just being confident and comfortable and happy with myself.

So after my surgery was the exciting process of recovery. So I remember them putting ice packs on me when I woke up and they wanted to keep the swelling down. I did not want the ice packs on me, so I remember being very difficult about that. But they were just very adamant about keeping that swelling down to prevent more swelling because it will swell. And so I went home. I remember getting in the car. And I remembered getting home and I remember taking pictures. I still have pictures. I was literally taking pictures of the angles and I sent them to my dad and then I remember I went to sleep. My mom gave me medication when it was time and then I went to sleep and I slept the entire night and then the next day I woke up and I didn’t have any bruising just yet. That takes a couple days, usually, to set in. But again, obsessive with taking pictures. I was just so excited. I’m looking at every little angle I had.

I also had the splints put in, which is usually that is when they have to do an open rhinoplasty generally, which I had to have because I had my deviated septum fixed as well. So I had this one’s put in, you go back three days after. So it was a little uncomfortable, but it really wasn’t that bad. I was taking my medication and just trying to rest. The second and third day is when the beautiful bruising started to come about and I bruise pretty easily. So I was expecting the worst. And it was actually kind of cool to see how the body works like that. I’ve never seen my body do anything like that and because it was for vain reason, I actually was like this is kind of neat, wasn’t something super traumatic. It was something I was expecting.

So I just remember taking pictures every day. I did a Snapchat for my friends even, and it was the third day, so my swelling was at its peak. I have pictures in my phone that I still love to show people of how bruised I was because it was crazy bruising, because again, I got the external and internal. Just kept trying to do ice packs and keep the swelling at bay, taking bromelain extract to help with the swelling as well. I needed the medication mostly the first couple of days, but then I tried to taper off a little bit because I was trying to be tough. And it honestly didn’t hurt as bad as I thought it would. It wasn’t a big deal really.

So recovery, when you wake up, you have … Well, I had internal splints and external splints. The internal splints are not awesome and you can have blood that drips out. I really honestly had minimal blood drip out. On day three they take the internal splints out, which is a little bit painful as they’re coming out and it is amazing once they’re out, and then I kept the external split for about a week. That came off. I had minimal bruising. I did not have to take one single pain pill. I did not have any pain at all and I don’t have a high tolerance for pain. You do have a little bit of nausea even the second day, but that’s just because you have all that blood during surgery that goes to your stomach.

Two weeks after surgery, it was New Year’s Eve. I went out with absolutely no makeup, no pain, no bruising, and people even now don’t realize that I had my nose done. They just say that I look great, which I think is amazing. Some of them will go, “Oh, your eyebrows look really good.” It’s like, no, I have a new nose, which speaks to his skill in making you look natural and not like you’ve had plastic surgery, which is amazing.

You’ll need to wear a cast on your nose for the first week after surgery. This will then be removed and the swelling will slowly subside over the following weeks. Usually after 2-3 weeks, the swelling will have subsided enough where people will not be able to tell you’ve had surgery. Some pain is to be expected in the days immediately following surgery although medication will be prescribed for this. It takes approximately 3-4 months for the swelling to completely subside when you have rhinoplasty. After this time you’ll be able to appreciate the final results.

Will I need to take time off work?

Yes. It’s usually recommended that you take a couple of weeks off work so you’re able to recover properly and don’t have to worry about your nose being accidentally bumped or knocked, which could cause complications.

RHINOPLASTY – Revision rhinoplasty techniques and secrets

Revision rhinoplasty is especially challenging. I tell my patients when they come in, it’s kind of like somebody sells you a 100-piece Lego set, but there are like 20 pieces missing and you’ve still got to make the picture that’s on the front of the box. So now you have to come up with a different plan. You’re not given the whole thing, yet you still need to create a great looking nose.

What we really love to have is as many records as possible. Previous operative notes, I know a lot of the rhinoplasty surgeons in my area, in my state, and actually regionally, nationally, so often I kind of know how many people approached the nose. So as much information as we have the better.

We also want to see why the untoward result happened. Was it being over aggressive? Was it maybe a plan that was doomed to fail from the beginning? So again, going back to the consultation process, we want to make sure that we create a realistic expectation of what might work.

Often during revision rhinoplasty, patients want a full redo, and unfortunately, like everything in life, you can’t have a full redo. Sometimes there are things that you can’t do, and that’s just as important as things that you can do. So often I’ll tell them, “Hey, pick three things that I can fix on your nose,” and there might be five things that bother them. Well, sometimes you can’t have all five. And this is one of those things that, again, in the consultation process we figure out.

Thankfully, though, because of experience and doing a lot of rhinoplasties and having a pretty deep bag of tricks when it comes to fixing noses, we can generally give the patient what they’re looking for. And again, going back to the main concepts, a nose that looks great, a nose that works great, and most importantly and nose that’s built to last.

So we go back, we see what we have. Sometimes it’s a Pandora’s box. The patient doesn’t know there are not operative notes, and we don’t know what’s there until the day of surgery when I’ve opened up the nose and I’m like, “Oh wow, surprise, there’s some stuff missing. We had no idea.”

So again, this is where experience comes in. You’re like, “Okay, on the fly, what would I do in this situation?” Again, having a deep bag of tricks, having the experience really allows me to say, “Okay, we’ll switch gears, we’ll figure it out,” and we’re often able to have a great solution to whatever problems that we encounter inside the surgery.

So although revision rhinoplasty is extremely challenging, it’s actually one of the most rewarding surgeries we can do. Because the patients come in, they’re obviously distraught, and they think life is over because they tried to get a good result and they ended up with something that nobody wanted. And we’re often able to deliver a great result, put it back together, happy patients, excellent result, and one of the reasons I love doing rhinoplasty.

REVISION RHINOPLASTY – Describe your sinus history and previous nose surgery

All right. So in high school I was a cheerleader, a big competition cheerleader. I was a base and I would be hit in the nose countless amount of times. That led to a deviated septum where I couldn’t breathe out of the left side of my nose. It was very difficult, very uncomfortable, never felt like I got proper air flow, didn’t sleep well. So, at that time I kind of went to go look for other procedures, things like that, that could help with nasal breathing and found a physician who did a septoplasty on me. Things went great. I could breathe better. It was awesome. Then I went back for my post op appointment and was told that my septum had been perforated.

Since then, I had chronic rhinitis. My nose was always running. I had a whistle through my nose. It was hard being in a quiet setting and talking to people, they could hear my whistle, and it was honestly not fun and it was a little embarrassing. So I came back to Dr. [Sinha 00:00:01:06], just realized that I was dealing with more functional issues rather than just the sinuses. That perforated septum was really causing a lot of issues, and so we talked about that and discussed it and decided that we needed to proceed with the functional endoscopic sinus surgery as well as to fix this perforation.

And at the same time, because of the perf, it was starting to make my nose kind of dip in a little bit, just slightly to where only I could notice it and he could notice it a little bit during the exam. But overall we decided that a functional rhinoplasty would be the best way to kind of fix and make it more structurally sound.

REVISION RHINOPLASTY – What was your day of surgery like?

So, day of surgery, it was pretty easy. I got up about 5:30 in the morning. That part wasn’t fun. But, went into the office and I had to get there around 6:30 or so. Surgery was at 7:30. Was brought back, placed in a room, started the postop medications. My mom was there with me the whole time. Honestly, IVs kind of freak me out a little bit, so I was a little nervous about that. But once I got the medications, I easily was relaxed. Everything was very organized. At one point I was put to sleep and went in, and next thing I knew I was waking up from surgery.

And I am a year and a half out now and looks really good. So, it was an overall pretty good process. Definitely a couple of hard days, but overall I wouldn’t change a thing.
